Eligibility & Enrollment

Before deciding whether you need to enroll, take a close look at all of the options offered. You may experience changes from year to year, and it’s a good idea to make sure your benefits still fit you — and that you’re not paying for more coverage than you need.

WHO IS ELIGIBLE?

Full-time team members working at least 30 hours a week are eligible for the benefits outlined in this Digital Benefits Hub. Visit usbenefits.plexus.com for more information about dependent eligibility.

Note: If your spouse is eligible for medical coverage elsewhere and they are covered under your Plexus medical plan, you will pay an extra $175 per month.

If you work less than 30 but more than 20 hours per week, you are eligible for Basic Life and Accidental Death and Dismemberment (AD&D) Insurance and the Plexus Corp. 401(k) Retirement Plan.

When can I enroll?

You can only enroll at certain times throughout the year.

  • As a team member, you can enroll during the annual Open Enrollment period.
  • If you are a newly hired team member or experience a qualifying life event, you must enroll or waive coverage within 31 days.

How do I make changes?

Open Enrollment is the one time each year team members can make changes to their benefit elections without a qualifying life event (QLE). You can enroll or make a change to your benefits during the year if you have a qualified life event such as: • Change in marital status, number of dependents, employment status impacting coverage, or a change in dependent eligibility • Medicare or Medicaid entitlement causing gain or loss of coverage • Gain or loss of other coverage through your spouse or for your dependent • Receipt of a Qualified Medical Child Support Order You may add or terminate coverage, change coverage levels (e.g., from single to family coverage), and add or remove dependents. You cannot change plans mid-year. Team members enrolling due to a QLE must enroll or waive coverage within 31 days from the date of the qualifying life event and provide supporting documentation.

ABOUT THIS DIGITAL BENEFITS HUB: Actual plan provisions for PLEXUS (“the Company”) benefits are contained in the appropriate plan documents, including the Summary Plan Description (SPD) and incorporated benefit/carrier booklets. The Digital Benefits Hub is a summary only and does not describe each benefit option. This Digital Benefits Hub provides updates to your existing SPD as of the first day of the plan year, which describes your health and welfare benefits in greater detail. Until the Company provides you with an updated SPD, this Digital Benefits Hub is intended to be a Summary of Material Modification (SMM) and should be retained with your records along with your SPD. As always, the official plan documents determine what benefits are available to you. If any discrepancy exists between this Digital Benefits Hub and the official documents, the official documents will prevail. The Company reserves the right to amend or terminate any of its plans or policies, make changes to the benefits, costs, and other provisions relative to benefits at any time with or without notice, subject to applicable law.
